Admissions
Our goal is to make admission to Independence House as convenient as possible for clients and families, for referring physicians and hospitals.

The staff at Independence House can properly admit a client any day of the week (including weekends) and almost any time of the day. For those clients who require transportation, it is available.

Working with referring physicians and hospitals - often receiving doctor orders by fax (419) 435-0829 - Independence House personnel can admit clients and arrange prescribed medications on short notice.

When possible, we encourage prospective clients and families to complete the Pre-admission form. After receiving a request for admission, the nursing staff will contact the client's physician and/or hospital for additional information needed to develop a nursing care plan.

Our policy is to be prepared to begin service the moment a new client enters the front door of Independence House.
