Accommodations
Independence House, in Fostoria Ohio, offers both private and semi-private accommodations. All rooms have private bathrooms.
Clients requiring Skilled Nursing Care in most cases will qualify for Medicare or other insurance which will cover the additional cost of skilled care. Medicare and most other insurance plans do not include the cost of a private room.
In case of hospitalization or leave of absence, the client or sponsor may hold the room by reservation at 50 percent of the regular rate.
The daily rate includes room, meals, nursing service, personal service, laundry, housekeeping, and activity programs. The day of admission is a billed day. The day of discharge is not billed if discharge takes place before noon.
Television, Telephone and Spa
All rooms are equipped with TVs and cablevision at no additional cost. All rooms have telephone lines. Clients may bring their phone sets from home or rent a phone from the facility.
A whirlpool and footbaths help to make our "spa" more comfortable.
We believe this kind of treatment is "holistic" because it benefits the whole person. It helps people relax, feel less anxious.
Payment
Prior to admission it is necessary to determine how the nursing home bill will be paid. If the admission is to be covered by Medicare, it is still necessary to arrange for the co-insurance payment, and that happens if the stay exceeds the Medicare period of coverage. Social Services is available to discuss Medicaid.
Independence House staff will assist in any reasonable manner with insurance claims. However, the client, staff or family is expected to arrange for payment unless Independence House has received confirmation from the insurance company that the nursing home stay is covered and that the payment will be made directly to the facility.
Someone should be designated as the Financial Responsible Party. This is the person who will receive the monthly bill and see that Independence House is paid. Unless covered by insurance, upon admission the daily room rate will be billed for the remaining days in the month. Thereafter, on the first of the month the entire month will be billed in advance.
Medicare
Independence House is certified by the Ohio Department of Health to provide skilled nursing care.
Independence House has a Medicare contract to provide skilled nursing and rehabilitation services to eligible clients.
Skilled care is defined by Medicare based upon rehabilitative
potential and medical complexity of the required nursing services.
It is not based upon the amount of care needed but on the skill level required in meeting the need. It is possible for a client whose condition is very serious and requires a maximum amount of service not to need skilled care.
We suggest clients with nursing home insurance and Medicare supplemental insurance
review these policies with the administrator to be certain all benefits are obtained.
Unless the facility has a contract with the insurance company to allow for direct billing, the client or family must pay for the care and receive reimbursement from the insurance company.
Independence House offers in-house physical therapy, occupational therapy and speech therapy programs.
Nondiscrimination
Independence House provides all services and employment to all persons without regard to race, color, national origin, handicap, or age. All persons and organizations having occasion either to refer persons for services or to recommend our services are advised to do so without regard to the person's race, color, national origin, handicap, or age.
Physician
All clients who are admitted to Independence House nursing home must be under the direct care of a physician who must provide written orders for all medications, treatments, and Plan of Care. The facility encourages the client's regular physician to continue this important role while a resident of Independence House.
Pharmacy
Independence House has a contract with Omnicare Pharmacy to provide the following services to our clients:
- Twenty-four hours per day delivery of medication including emergency delivery.
- Monthly-computerized individual medication record.
- Drug interaction check to ensure that medications can be taken together.
- Individual medication packaging, unit dose.
- Monthly medication review on every client conducted by a pharmacist.
- Nurse follow-through with physicians and pharmacy on all medication orders and changes.
For most clients, the unit dose system is preferred. For those who choose to use another pharmacy, there is an additional per day charge. Some clients may be able to self-administer their own medications.
